Objective: To evaluate the sensitivity and specificity of palpation and percussion for splenic enlargement, as the accuracy of many of the clinical maneuvers we perform remains largely unstudied. Methods: One hundred cases were selected at random from medical wards, and splenic enlargement was assessed by palpation and percussion of the Traube's space Barkuns method , which was confirmed by the ultrasonography of the abdomen. The results of the various tests were tabulated and assessed statistically. It was also found that the BMI of the false negative cases was significantly higher And maximum clinical utility is achieved when both percussion and palpation are combined.

Percussion of Traube's Space--A Useful Index of Splenic Enlargement

Traube's semilunar space is an anatomic space of some clinical importance. It is a crescent-shaped space, encompassed by the lower edge of the left lung , the anterior border of the spleen , the left costal margin and the inferior margin of the left lobe of the liver. Thus, its surface markings are respectively the left sixth rib superiorly, the left mid axillary line laterally, and the left costal margin inferiorly. The normal human spleen measures about millimeters in length, and splenomegaly is an important clinical sign. There are 2 possibilities to evaluate splenomegaly in the clinical examination: percussion and palpation. Beneath Traube's space lies the stomach , which produces a tympanic sound on percussion.
